To be used by all members to notify PERA of a change of address, a change of name or a change of social security number. You can now update your mailing address, phone number and email address in your PERA RIO account. Please visit https://perass.state.nm.us/SelfService/ to update this information rather than submitting a Change In PERA Records form. Updated April 2025
